Levels of Hospice Care

Rejuvenate Hospice offers comprehensive hospice services, including routine home care, continuous care, inpatient care, and respite care. Each level is designed to address the specific needs of patients and families at different stages of the journey. Our team works closely with you to ensure seamless transitions between levels of care as needed, always focused on comfort, dignity, and quality of life. Wherever care is provided, we are committed to bringing peace, support, and understanding.

The four levels of hospice care are as follows:

1. Routine Home Care This is the most common level of hospice care, provided wherever the patient calls home — whether a private residence, assisted living, or nursing facility. Our team offers regular visits to manage symptoms, provide comfort, and support both patients and families.
2. Continuous Care Continuous care is provided during times of medical crisis when a patient’s symptoms require round-the-clock nursing care at home. Our team stays with the patient to ensure comfort and stability until symptoms are under control.
3. General Inpatient Care When pain or symptoms cannot be managed at home, patients may receive care in a hospice facility, hospital, or nursing home. Our team provides intensive medical and nursing support to bring relief and comfort.
4. Respite Care Respite care provides temporary relief for primary caregivers. The patient is cared for in a hospice facility or nursing home for up to five days, allowing loved ones a much-needed break while knowing their family member is safe and cared for.
